Based on the reason selected, the following documents must be uploaded for your request to be reviewed.

Proof of payment – If paid by:
- Cash or Check: A clear copy of your receipt or the front and back of your cancelled check.
- Money Order: A clear copy of the front and back of your canceled money order. In order to retrieve this, you must take your receipt to the place of purchase and request that a tracer be placed on the money order. We do not need the non-negotiable customer receipt.
- Credit Card: A clear copy of your credit card statement that shows the last four digits of the credit card number and payment to the Philadelphia Parking Authority.
